insurance certificate, resident-entry notice or promise that every item is included.

PROPERTY & AUTHORIZATION

[ ] Number of units, HVAC systems and dryer exhaust lines, if known
[ ] Individual-unit equipment versus shared or common equipment
[ ] Management approval and resident access arrangements

SYSTEMS & ACCESS
[ ] Duct/HVAC components requested: supply, return, plenums, blower, coil, filters
[ ] Dryer routing: individual lines, shared manifold or riser, wall or roof outlets
[ ] Roof/mechanical-room access; service elevators, loading and parking
[ ] Known damage, inaccessible lines or suspected hazardous materials
[ ] Preferred work windows and operating-hours constraints

WRITTEN PROPOSAL
[ ] Included components and excluded work; optional items priced separately
[ ] Proposed cleaning methods suited to the equipment and duct material
[ ] Total price, additional-work approval and cancellation/rescheduling terms
[ ] Vendor insurance requirements reviewed; available COI/endorsements confirmed
[ ] Any requested photos, unit log or completion notes explicitly included

CLOSEOUT
[ ] Systems or lines serviced, inaccessible areas and follow-up work recorded
[ ] Documentation received for the agreed scope
[ ] Next inspection interval based on use, equipment guidance and actual conditions
